Normally, the dental bone graft material used is a type of calcium phosphate, as this imitates natural bone and is a biodegradable material, which is safe to be used in the body. This technique has many other advantages. For example, the dentist can obtain precisely the right amount of bone to graft successfully. Also, there is no risk of disease transfer. Also, the donor’s and the recipient’s blood types do not need to be identical. The advantage of synthetic material is that it has no living cells, so the risk of rejection is minimal. Its only disadvantage is that the body might reject the bone graft in the worst case.

What is the best bone graft material?

There is no single answer – and anyone who gives you one without having seen a CBCT scan should make you sceptical. Whether autogenous, allogeneic, xenogeneic, or alloplastic bone graft material is used depends on several factors: the size and location of the bone defect, the patient’s general health, and – something many forget – whether a tooth needs to be extracted anyway. If an extraction is necessary, the extracted tooth can, in some cases, be processed directly as an autogenous dentin graft – extraction and bone augmentation in a single session, without a separate donor site. This is biologically active, accepted by the body, and particularly convenient for dental tourists because it saves on travel. Xenogeneic materials such as bovine hydroxyapatite have also proven effective for smaller defects, while alloplastic materials score points for their standardised availability. The honest answer is: the best bone substitute material is the one that suits the clinical situation, not the most expensive, not the most well-known.
